Overview

Summary:

The Physical Therapist acts as a care manager and is responsible for overseeing and ensuring a comprehensive rehabilitation program designed to meet the physical, psychosocial, and immediate environmental well-being of the patient. This includes promoting client/caregiver ability for self-determination and care, optimizing their participation in this process and optimizing outside resource utilization to achieve self-management. The Physical Therapist is responsible for coordinating and communicating with various disciplines or services involved in client care and coordinating their efforts toward discharge goals established at onset and during course of care. Is a patient advocate.

Qualifications

Other information:

Bachelors degree and educational requirements needed to be licensed as a Physical Therapist in New Mexico and to meet requirements of Home Health Medicare Conditions of Participation if necessary.

One year of experience in supervised clinical practice in acute care or rehab setting preferred.

Basic typing competency preferred.

Need to have current BLS certification, NM drivers license as well as auto insurance.

Responsibilities

Responsibilities:

  • Evaluates patients, develops plans and goals, and implements treatment programs for improvement of physical or developmental dysfunction and independence in activities of daily living. Continuously reassess treatment plans and goals.
  • Evaluates, observes and instructs patients on functional skills. Educates and instructs regarding home programs and preventive measures as appropriate to patient, family, and others. Leaves written instructions in the home.
  • Must demonstrate knowledge of the principles of growth and development over the life span of patients served
  • Initiates documentation in the home and completely records all care delivery as required by the profession and the department by the end of each day; this includes admissions.
  • Communicates with patient, family, physician/Advanced Practice Clinician (APC), and other healthcare providers regarding patient care.
  • Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
  • Participates in weekly team conferences, educational programs, in-services and other meetings as required. Participates on special project teams,
  • Participates in agency continuous quality improvement activities as required
  • Maintains a department defined productivity standard for visits and caseload management.
  • Precepts newly hired therapists and students and serves as a resource person and professional role model as needed.
  • Provides supervision and oversight of Physical Therapy Assistants and Home Health Aides/Certified Nurse Assistants as required by regulation.
  • Responsible for holiday and weekend coverage for agency admissions and patient home visits as required on a rotating basis.
  • Performs other functions as required.
