World Relief seeks a Services to Older Refugees Case Worker to provide case management services to refugees and immigrants that are 65 years and older to help them thrive in the Quad Cities community after their initial resettlement period. This position would serve those who have been in the US between 6-50 months who are looking for help with social interaction, education, physical or mental healthcare, benefits applications, immigration status changes or other concerns by continuing to guide them through navigating US systems.

Role & Responsibilities

  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Recruit and screen individuals who are 65 or older to participate in senior programming designed to help them increase social connections to reduce social isolation
  • Collaborate with client to make an appropriate service plan to address their needs
  • Conduct education sessions relevant to community resources and systems in the US regarding the elderly population
  • Provide opportunities for social interaction in a communal environment
  • Conduct in-home assessments with qualifying clients to assess specific needs
  • Provide case management services in accordance with grant agreements and tailored to each client’s individual needs
  • Assist clients in navigating healthcare systems, including making referrals to relevant specialists or primary care providers
  • Assist clients with updating their immigration status and making referrals to apply for permanent residency or citizenship, as well as relevant medical providers regarding immigration-related medical concerns
  • Assist clients in applying for disability benefits, age-related SSDI, or other medical benefits including in-home medical care
  • Connect program participants to appropriate ESL opportunities
  • Educate family members on resources available to assist with age-related care and benefits
  • Determine adequate referrals to long-term services or intensive case management for clients with vulnerabilities
  • Conduct regular follow-ups to ensure that seniors receive the care they are guaranteed with the Older Americans Act 
  • Create and maintain accurate and organized case files
  • Work with colleagues/interns, resettlement partners, volunteers, and World Relief staff to ensure that client’s needs are met in a planned, effective, and timely manner, while honoring confidentiality
  • Collaborate with other World Relief staff through daily communication and weekly meetings
  • Perform all duties in a culturally and linguistically appropriate manner

Knowledge, Skills, & Abilities

  • Strong interpersonal communication and conflict resolution skills
  • Strong organizational skills and ability to maintain detailed, accurate records
  • Experience and knowledge in applying for public benefits
  • Knowledge of local community resources
  • Task oriented with excellent time management and problem-solving skills

  • While performing the duties of this job, the employee is required to have ordinary ambulatory skills sufficient to visit other locations
