Job Description Summary:

Performs duties expected of a staff pharmacist.

Leads medication safety strategy for accountable site(s) in collaboration with system medication safety team.

Assists in fulfillment of team member staffing vacancies (e.g., TAP) when necessary.

Facilitates and manages activities of the hospital’s Medication Safety Committee.

Utilizing medication event-reporting data, published best practice standards, and published events or recommendations, identifies opportunities for proactive change in the medication-use system to improve patient safety.

Responsible for ensuring compliance with national patient safety recommendations and regulatory standards related to safe medication use.

When a significant medication error occurs, participates in the root cause analysis evaluation and recommends system changes as necessary.

Leads and participates in failure mode and effects analysis on medication use systems and performance improvement projects as necessary.

Reviews high volume, high risk, and problem prone or critical aspects of care processes for prioritization of potential medication safety improvements or initiatives.

Ensures that healthcare providers are educated in safe medication-use practices. Collaborates with different departments and committees focusing on patient safety and serves a preceptor to Pharm.D. candidates and pharmacy residents

Integrates medication safety improvement initiatives, activities and changes with clinical and operational department programs and committees (i.e., P & T Committee, Patient Safety Council, and Pharmacy/Nursing Committee).

Minimum Qualifications:

Bachelor’s Degree (Required)OPL – Ohio Pharmacists License – Ohio Board of PharmacyOhio Board of PharmacyOhio Board of Pharmacy

BS or Doctor of Pharmacy degree from an accredited institution and a current Ohio pharmacist license. Completion of a Pharmacy Practice residency or equivalent hospital Experience . Pharmacology/Therapeutics for all age groups. Aseptic Technique and drug literature review. Previous hospital pharmacy Experience .
