Visit dignityhealth.org/northridgehospital for more information.Community Health Navigator – Community & School Health InitiativeNorthridge Hospital Medical CenterNorthridge Hospital Medical Center is seeking a passionate and dedicated Community Health Navigator to join our Community Health Work & Sustainable Outreach and Navigation (CHW SON) team. This position is grant-funded and will play a vital role for outreach and engagement to provide direct referrals to services for people who were the most affected by the pandemic. The goal is to further reduce health disparities by addressing social determinants of health, offering early intervention, and promoting health education.
Responsibilities
- Proactively reach out to community members and their families to provide information and support.
- Conduct initial assessments to understand individual needs and connect them to community resources.
- Educate community member on COVID-19 and other health related issues through tabling events, presentations, and workshops
- Build rapport and establish trusting relationships with community members and their families.
- Advocate for community members needs and rights, ensuring they receive timely and appropriate care.
- Provide emotional support and resources to community members and their families.
- Connect community members with relevant resources, such as financial assistance programs, transportation services, health, mental health and social services.
- Maintain accurate records of community members interactions and outcomes.
- Contribute to program evaluation and reporting activities.
- Other duties as assigned.
Qualifications
- BS degree or equivalent college education and work experience preferred
- 6 months of experience working with the public or with community groups.
- Excellent verbal and written communication skills and information management skills required.
- Excellent organizational skills required.
- Bilingual English / SpanishÂ
- CA Driver License
- Proof of Insurance
- Fire Card (must obtain within 90 days of hire date. We offer the classes in the hospital)
