Position Overview:
Veritas Health Services is seeking a motivated and compassionate Health Coach to join our team. In this role, you will provide support and guidance to patients in achieving their health and wellness goals, promoting a healthier lifestyle through education and personalized coaching.

Key Responsibilities:

• Conduct assessments to identify patients health and wellness needs and goals.
• Develop and implement personalized health coaching plans tailored to individual patients.
• Provide ongoing support, motivation, and accountability to patients through regular check-ins.
• Educate patients on nutrition, exercise, and lifestyle changes that promote health.
• Collaborate with healthcare professionals to ensure a holistic approach to patient care.
• Maintain accurate records of patient progress and adapt plans as needed.

Qualifications:

• Minimum of 12 months of experience in health coaching or a related field.
• Strong understanding of health and wellness principles.
• Excellent communication and interpersonal skills.
• Ability to motivate and inspire patients to achieve their goals.
• Knowledge of behavior change strategies and techniques is a plus.

Requirements:
