POSITION SUMMARY:

To be fully engaged in providing No Harm/ Quality, Customer Experience, and Stewardship, the offsite pharmacist coordinates assigned consultant pharmacist functions, including but not limited to, Health First Aging, medication management and inspections at the Melbourne Same Day Surgery, Melbourne GI Center, HRMC Pain Management, HRMC Wound Care and Hyperbarics Center, Medical Rehabilitation, Diagnostic Centers, Health First Distribution Centers, and General Stores. The Consultant Pharmacist shall also monitor Joint Commission readiness for said entities, and provide the hospital staff, medical staff, and patients with accurate, current information concerning medication therapies.

PRIMARY ACCOUNTABILITIES:

Quality/No Harm:

  • Performs medication inspections for offsite facilities.
  • Prepares medication for use in stated facilities as needed
  • Ensures State and Joint Commission inspection readiness for offsite facilities. 
  • Assists with documentation of medication variance reporting.
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
  • Detects and reports suspected adverse drug reactions accurately and in a timely manner.
  • Provides accurate, adequate, and timely medication safety and drug information to the professional staff.
  • Documents all clinical activities and interventions accurately and completely.
  • Prepares and participates in the Joint Commission and ACHA inspections.
  • Completes all competency/skill assessment requirements (see attached competence assessment/skill list).
  • Maintains current pharmacy and consultant licensure.
  • Prepares quarterly reports for inspections.
  • Apprises the director and pharmacy manager of the status of all projects involving Joint Commission and State compliance issues.
  • Assigns and maintains QI/QC activities.

Customer Experience:

  • Serves as a liaison to the Health First Offsite facilities.
  • Provides accurate, adequate, and timely drug information to the professional staff.
  • Participates in the development and presentation of orientation, education, and training programs to the staff.
  • Participates in the Pharmacy and Therapeutic Committee, as needed.
  • Attends pharmacy staff meetings and reads pertinent email communication.
  • Pharmacists who are certified to administer, per Florida law, may administer influenza, pneumonia, and/or varicella zoster vaccines to Health First associates and patients.
  • Attends orientation, education, and training programs. Reviews literature and other materials pertinent to the practice of pharmacy.
  • Performs other duties as assigned by Director.
  • Maintains patient and department/hospital confidentiality.

Stewardship:

  • Maintains policies and procedures that promote cost-effective, appropriate, and safe medication use. 
  • Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic protocols and promoting rational drug therapy selection.
  • Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost effective therapeutic alternatives to medical staff, as needed.

Qualifications

QUALIFICATIONS REQUIRED:

  • Must be a graduate of accredited College of Pharmacy with a BS Pharmacy degree, Pharm D preferred.
  • Must be licensed to practice in State of Florida by Board of Pharmacy.
  • Must be licensed as a Consultant Pharmacist by State of Florida Board of Pharmacy.
  • Must have at least two years of hospital pharmacist experience.
  • At least one year of The Joint Commission inspection experience preferred.
  • Hours as needed to fulfill consultative responsibilities.
  • Licensed Healthcare Risk Management Certificate preferred but not required.

PHYSICAL DEMANDS:

  • Repetitive use of hands and fingers. Good hand and wrist dexterity to enable typing and/or computer work for 8-10 hours/day.
  • Good eyesight to read labels/prescriptions, graduated containers of fluids, to count tabs, capsules.
  • Good hearing to understand phone instructions/orders, customers and those in which the position depends.
  • Ability to work in noisy distracting atmosphere, to handle frequent interruptions, to adapt to changes in workload and work schedule.
  • Handle large volumes of work for long periods of time.
  • Walk, bend, stoop, twist, sit, stand for long periods of time, 8-10 hours a day.
  • May require lifting and carrying light loads (Up to 40lbs.).
