Pay Range: $19.33 – $26.57

JOB PURPOSE: The Dental Health Home Coordinator is responsible for achieving the TRIPLE AIM by improving the population’s health, our patients’ experience of care; all while decreasing costs. As a liaison between the patient, El Rio Health’s Medical and Dental service lines, other medical entities, HMOs, and inpatient facilities, the Dental Health Home Coordinator will ensure that the coordination of health care services, education, and case management, align with the patient’s health care needs with special focus on oral health treatment coordination. Additionally, the Dental Health Home Coordinator will collaborate with patients, medical and dental treatment teams, Health care providers, and other community services to manage population health data of the assigned patient panel and reduce or eliminate care gaps to improve health outcomes. The Dental Health Home Coordinator will provide education to the patient that is pertinent to the patient’s condition and that empower the patient to make good health decisions.

The primary goal of the El Rio Health Dental Health Home Coordinator is to support El Rio’s Mission of providing comprehensive, quality health care that is affordable and accessible to all who may have healthcare needs, by successfully performing the primary essential functions.

Essential Job Functions:

  • Serves as the primary point of contact between patient, medical and dental treatment teams, and internal/external stakeholders to ensure continuity of care.
  • Supports patient in navigating initial health center dental appointment to ensure positive patient experience.
  • Facilitates/assists patients in the new patient intake process.
  • Maintains knowledge of the link between oral health and common chronic health conditions, behavioral health disorders and management techniques to enhance patient “self-care.”
  • Provides ongoing direct support services to patients with goal of improving and/or maintaining whole health.
  • Serves as advocate for patients to ensure their oral health needs are met and to role model self-management skills.
  • Maintains regular contact with assigned dental patients to monitor progress, identify any service needs and communicate concerns to medical and dental treatment team.
  • Helps patient with financial responsibility review of oral health treatment plan and presents oral health treatment plan to the patient.
  • Facilitates treatment plan presentation to patient.
  • Educates patient on treatment options.
  • Reviews financial responsibility with the patient.
  • Facilitates referrals, assists in scheduling appointments, assesses eligibility for/secure enrollment in appropriate health insurance coverage for patient and discuss financial arrangements with patient.
  • Supports and navigates patients in securing and enrolling for specialty dental services as needed.
  • Partners with assigned medical and dental provider(s) and treatment team to coordinate care and follow up on any urgent needs of patients.
  • Assists patients with enrollment into and usage of the patient portal.
  • Facilitates communication and coordination with specialty dental services and ensures appropriate planning on behalf of patient.
  • Monitors and evaluates data related to gaps in dental care of assigned patient panel; and utilizes analytical software to manage population health data of assigned patient panel.
  • Maintains a clean, safe, and hygienic work environment in compliance with all Policies and Procedures including but not limited to work areas, workstations, examination rooms, hand washing, infection prevention and control etc. for this position.
  • Demonstrates an understanding of and proficiency with the application of all compliance and reporting requirements respective to Joint Commission Certification (JCC) standards.

Minimum Education and Experience:

  • Associate of Arts degree in Health Care, Human Services, or related field from an accredited college; Three (3) years’ experience with direct patient contact either in a dental clinic or health care setting 
  • Three (3) years’ experience working with Electronic Health Records (EHR).

If applicable, equivalent combination of education and experience may be considered, and must be directly related to the functions and responsibilities of the job.

Required Licenses, Certifications, and Registrations:

  • Certificate as a Dental Assistant or a Medical Assistant from an accredited program.
  • Current certification in Basic Life Support (BLS) for health care providers.
  • Level I fingerprint clearance card: current valid and in good standing or have applied for it within seven working days after beginning employment.
  • Employees in this position are required to have reliable transportation that can meet any operational, reassignments of the organization during the workday. If an employee is driving during work hours, the employee is required to possess a valid driver’s license and must be in compliance with Arizona vehicle insurance requirements.

Preferred Education, Experience, Skills, Abilities:

  • Certified Application Counselor (CAC) Program.
  • Bachelor’s Degree in Health Care, Human Services, or in a related field from an accredited college.
  • Five (5) years of experience working with Electronic Health Records (EHR).
  • Five (5) years of experience working with direct patient contact either in a dental clinic or health care setting.

Bilingual (English/Spanish) with the ability to speak, read and write in both languages.

All employees are required to undergo drug testing prior to employment and will be subject to post-accident, reasonable suspicion, return to duty and follow up drug and alcohol testing in compliance with Federal and State regulations for alcohol and controlled substance testing.
