Department Overview

The Hemostasis and Thrombosis Center (HTC) is home to over 45 faculty and staff, engaged in the clinical, research, public health, outreach and education missions. The HTC is a multidisciplinary center, providing comprehensive care to adult, adolescent and pediatric patients.

Function/Duties of Position

This position is responsible for providing the services of a licensed pharmacist within the HTC. The pharmacist specializes in caring for patients of all ages with hemophilia and other bleeding disorders in an ambulatory/outpatient pharmacy setting. The pharmacist in this role actively participates in patient visits, provides direct patient care, and collaborates closely with prescribers and the interdisciplinary clinic team. Responsibilities include medication therapy management, ensuring that prescribed treatments are appropriately coordinated and optimized to meet the needs of each patient.

This position also participates in prescription verification, dispensing, and other outpatient pharmacy functions to ensure the safe and accurate distribution of medications. Additional responsibilities include patient education on treatment regimens, self-administration techniques, and adherence strategies.

This position supports the mission, initiatives, and goals of the HTC clinic, ensuring alignment with its commitment to comprehensive, patient-centered care.

Required Qualifications

  • B.S. Pharmacy or Pharm.D.
  • State of Oregon Board of Pharmacy Pharmacist licensure.
  • Community pharmacy practice or inpatient pharmacy practice experience.
  • Proficiency in clinical drug therapy management (dosing, indications, contraindications, interactions, monitoring).
  • Ability to accurately evaluate and verify prescription orders for appropriateness, safety, and completeness.
  • Knowledge of contemporary medication-use systems (order entry/verification, dispensing, inventory management, and regulatory compliance).
  • Strong problem-solving and decision-making skills; ability to work independently with miniimal supervision.
  • Effective communication and interpersonal skills.
  • Competency in patient interviewing and teaching skills.

Preferred Qualifications

  • Specialist Certifications (BCPS, BCOPS, etc.).
  • State of Oregon Pharmacy Preceptor licensure.
  • Advanced pharmacy practice residency.
  • Experience in specialty pharmacy practice, including hemophilia/bleeding disorders either inpatient or outpatient.
  • Experience working in an ambulatory care or clinic-based pharmacy setting, particularly within a multidisciplinary care team.
  • Experience in managing patients with bleeding disorders.
  • Experience with factor product prescriptions.
