The Clinical Pharmacist implements Monogram’s medication therapy management (MTM) program, which ensures appropriate selection and use of medications by our patients. The Clinical Pharmacist will perform timely written comprehensive medication reviews (CMRs) by utilizing patient-provided medication lists, prescription claims and available clinical information (lab values, etc.). The pharmacist will work collaboratively with their local Market Physician Executive (POD physician) and clinical staff (e.g. Advance Practice Providers (APPs), nurses and social workers) to integrate medication management into Monogram Health’s overall care model and to ensure that field clinicians are deploying practices to optimize patient medication use and outcomes.
