I Accountability Objectives:
Under the direction of the Clinical Pharmacy Manager, provides pharmaceutical care including primary and
consultative services to patients and healthcare providers to ensure optimal drug utilization in the assigned specialty
area. Participates in clinical research, oversees and/or participates in the facilitation, preparation and dispensing of
medications. Provides education and training to pharmacy associates, Pharmacy students and Pharmacy Practice
Residents. Develops and continuously improves the provision of effective drug therapy services to all customers of
the Department of Pharmacy and those of St. Joseph Mercy Oakland in the designated specialty areas.
II Position Qualifications:
Minimum Education, Licensure / Certification and Experience Required.
A. Education
- Doctor of Pharmacy Degree from an accredited College of Pharmacy
B. Licensure / Certification
- Registered Pharmacist with a valid State of Michigan Pharmacist, Controlled Substance and Preceptor
- License
- Current Basic Life Support (BLS) certification.
- Current Advanced Cardiac Life Support (ACLS) certification.
C Special Skill / Aptitudes
- Excellent customer service orientation skills necessary in order to deal effectively with various levels of hospital personnel, outside customers and community groups.
- Possesses a good clinical knowledge base in the areas of pharmacology, pharmacoeconomics, pharmacokinetics and therapeutics and have a good knowledge of the principles of rational , efficacious, and cost effective medication therapy.
- Possesses professional and technical competence in the evaluation, criteria selection and utilization of pharmaceutical, medical, and other scientific literature.
- Previous teaching experience or teaching certificate for instructing or precepting pharmacists, pharmacy students, pharmacy residents, medical and nursing staff.
- Possesses excellent customer service orientation skills necessary in order to deal effectively with hospital associates, patient and patient families and outside community groups
D. Experience
- Completion of an approved ASHP or ACCP • PGY2 critical care or emergency medicine residency.
III Duties / Responsibilities:
- Provides direct patient care through patient care rounding, medication order review, medication therapy monitoring and consults with other healthcare providers to ensure the most effective use of medications.
- Provides medication procurement and dose preparation based on need and urgency with which medication is needed to meet the needs of patients and to comply with hospital clinical quality core measures.
- Leads/promotes a continuous quality improvement programs to meet Organizational, Emergency and Pharmacy Department goals. Monitors procedures and processes to identify areas in need of improvement.
- Prepares reports, educates and makes suggestions and recommendations for improvement to the appropriate leader or responsible Committee.
- Develops and monitors guidelines or drug use protocols specific to Emergency Medicine and the Trauma Service.
- Participates in resuscitations in the Emergency Department by preparing needed medications, determining appropriate medication therapy and ensuring correct dosing and administration of medication.
- Provides pharmaceutical care to boarded patients waiting for inpatient beds and participates in medication profile review for patients receiving high risk medications and patients at high risk.
- Assists the nursing staff with medication reconciliation for complex 7 and high risk patients
- Leads/participates in ongoing clinical research and other scholarly activities as appropriate.
- Participates in evaluating medications for formulary considerations with a pharmacoeconomic analysis and prescribing guidelines in conjunction with the Emergency Medicine Staff Physician. Conducts MUEs as necessary and recommends cost effective drug use.
- Precepts undergraduate Doctor of Pharmacy students for college of pharmacy clinical experiential experiences as well as precepts Pharmacy Practice Residents in emergency medicine clinical rotations.
- Works periodically in the pharmacy distributive operations and directly supervises pharmacy technicians in distributive functions.
- Provides/participates in clinical case conferences, continuing education programs, and other staff development programs in the Emergency Department or in the pharmacy department or with other Medical or Nursing healthcare providers.
- Leads/participates in determination of emergency drug needs for ambulance providers and works closely with the Oakland County Emergency Authority. Participates in emergency preparedness drills and leads the pharmacy department planning for emergency preparedness.
- Monitors regulatory compliance for medication management in the Emergency Department, documents and consults/makes recommendations to ER and Pharmacy leadership to maintain compliance.
- Reviews medication therapy for Senior ER patients presenting with a fall, mental status changes or those patients identified to need a medication review due to polypharmacy concerns.
- Participates in monitoring and ensuring medication safety in the Emergency Department. through proactive and continuous monitoring of medication practices. Identifies high risk medications or patients for monitoring for potential errors. Contributes to improving medication safety through root cause analysis and identification of potential solutions and policy/guideline development in conjunction with the other health care providers.

- Participates in the clinical training and orientation of newly hired pharmacists, pharmacy students and pharmacy residents and pharmacy technicians
- Leads/participates in appropriate departmental • and hospital committees.
- Maintains knowledge in specialty area by reviewing current developments in the medical or pharmacy literature, attending seminars, and involvement in regional professional societies.
- Establishes collaborative practice relationships with other healthcare providers in specialty practice area to enhance patient outcomes.
- Identifies and resolves operational and clinical service problems in collaboration with other pharmacy managers, coordinators, and specialists.
- Meets performance expectations for staff pharmacists duties and responsibilities in addition to duties and responsibilities identified in this job description.
- Assists in ensuring that pharmacy services are conducted in accordance with accepted professional standards of practice and organizational mission and values and integrity.
