Position Overview:

The Advanced Practice Clinician (APC) will provide oversight and clinical expertise to support innovation initiatives, including but not limited to, chronic disease management, home monitoring, and patient-centered hospital experiences. A key responsibility will be designing clinical workflows alongside our Chief Clinical Innovation Officer. This role will involve close collaboration with health coaches, Pharmacists, Primary Care Physicians, and Product teams to create digitally integrated care models that enhance health outcomes. The role will work with centralized digital operations to implement strategic plans that drive continuous innovation and improve care across multiple chronic disease programs.

Job Description:

EDUCATION:

  • Doctorate: Doctor of Medicine (MD), Doctor of Osteopathy (DO)
  • OR Master’s: Physician Assistant (PA)
  • OR Advanced Practice Clinician (APC)

CERTIFICATION & LICENSURE:

  • MD-Doctor of Medicine
  • OR DO-Doctor of Osteopathy of California
  • OR PA-Physician Assistant
  • OR NP-Nurse Practitioner

TYPICAL EXPERIENCE:

  • 5 years recent relevant experience
  • Digital health experience preferred
  • Experience understanding of primary care standards preferred
  • Expertise in clinical standards of primary care and chronic disease preferred.
  • Working knowledge of healthcare delivery across the continuum of care, and awareness of current issues and trends in health care delivery/operations preferred.

SKILLS AND KNOWLEDGE:

  • Working knowledge of healthcare delivery across the continuum of care, and awareness of current issues and trends in health care delivery/operations.
  • Knowledge and application of quality performance/process improvement methodologies e.g., Lean/Six Sigma, Plan-Do-Study-Act (PDSA), Rapid Cycle, etc.
  • Critical thinking and analytical skills with a keen attention to details.
  • Ability to define issues, collect data, establish facts and draw valid conclusions.
  • Displays a customer service focus in all decisions and actions.
  • Demonstrates a proactive approach in identifying and addressing issues and concerns.
  • Collaboration skills to facilitate open sharing of information and cooperation with various stakeholders to problem solve and achieve desired results/outcome.
  • Organization skills to effectively manage and/or re-prioritize activities and projects to meet deadlines while maintaining a high degree of responsiveness.
  • Ability to communicate through verbal and written means, and to present clinical and technical information to a variety of audiences/forums.
  • Ability to interact and maintain effective working relationships with those contacted in the performance of required duties.
  • Demonstrates respect for cultural and linguistic differences and promotes an inclusive work environment.
  • Ability to work effectively in a dynamic and fast-paced environment with changing business priorities.
  • Demonstrates initiative in providing feedback/input to improve workflow/processes.
  • Ability to maintain and work discreetly with confidential information.

Pay Range is $153.74 to $245.98 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
