APRN: Nationally certified licensed healthcare provider who provides primary and/or specialty nursing and/or medical care in a variety of healthcare settings. Functions in collaboration with a physician according to a standard care arrangement and specialty certification. Provides advanced nursing care to patients with complex acute, critical, and chronic health conditions, including the delivery of acute care services. Practice with a variety of specialty-based populations including neonatal, pediatric, and adult. Practices in any setting in which patients require complex monitoring and therapies, high intensity nursing intervention, or continuous nursing vigilance within the range of high-acuity care.
PA: Nationally certified licensed healthcare provider who provides primary and or specialty medical care (the practice of medicine) in a variety of healthcare settings. Functions the collaboration agreement of a Physician, (as per O.R.C. 4730) performs complete histories and physicals on patients of all ages to establish and record patient’s current health status and to develop a working diagnosis and treatment plan to provide continuing medical care, including daily examinations, writing orders, chart notes, discharging patients and completing discharge summaries. This medical care may be in a variety of clinical settings. PA’s are privileged by the Board of Governors. PA’s in this role provide advanced care to patients with in any setting in which patients require complex, acute, critical and chronic health conditions. Practice in a setting that requires complex monitoring and therapies and high intensity medical/surgical interventions and/or continuous vigilance within the range of high-acuity care.
Cleveland Clinic Primary Care Institute and Center of Connected Care is looking to add a Physician Assistant or Nurse Practitioner (Adult or Acute Care) to our Medical Care at Home APP team. This position works with the Medical Care at Home and the High-Risk Transitional Care Programs. Job responsibilities in these roles include but are not limited to in-home medical visits, managing complex high-risk chronic disease management, and working with a multidisciplinary team to prevent hospital readmissions.
This position is full-time, requires travel/in-home patient evaluation. Covering East-Side of Cleveland.
Typical schedule is Monday-Friday 8am-5pm.Weekend and Holiday coverage required on a rotating basis for Community Monitoring coverage.
Prefer providers with 1-2 years of clinical APP experience but will consider new graduates with home care experience.

Effective 05/01/2014: MSN required upon hire or within 3 years of hire for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role.
Graduate of an accredited APRN program.
If in neonatal care service area Master’s Degree is required upon hire.
PA
Graduate of an A.R.C. approved Physician Assistant Program.
Master’s degree in Physician Assistant Studies and/or higher degree obtained from a program recognized by their practicing states Licensing Board.
