Pharmacist in Erie, Pennsylvania

UPMC Hamot is looking for a full-time Pharmacist to join our innovative pharmacy team, working variable day and evening shifts, every third weekend, and three out of seven holidays per year. Hospital pharmacy experience as well as PA and NY state licensure are preferred. This position offers cross-training across all pharmacy areas, featuring state-of-the-art pharmacy automation and fully compliant USP compounding areas with advanced IV room technology. You will play a key role in verifying orders for Northern Tier UPMC Hospitals and supporting day, evening, and weekend staffing. As a self-starter, you will also participate in internal committee meetings and interdepartmental opportunities. In this role, there is the opportunity to –

• Interpret and evaluate physician orders to prepare and dispense appropriate medications in compliance with regulations.

• Identify and report adverse drug reactions in a timely and accurate manner.

• Sustain formulary management by minimizing non-formulary procurements and promoting rational drug therapy.

• Review and screen patient medication records for allergies, interactions, and therapeutic duplications.

• Organize daily operational tasks and supervise pharmacy technicians, aides, and interns.

• Adhere to all regulatory requirements, professional standards, and hospital quality management policies.

• Communicate with physicians and nurses to clarify unclear, illegible, or incomplete orders.

• Provide drug information and educational programs to healthcare professionals and pharmacy students.

• Utilize computer systems and technologies to enhance the provision of pharmaceutical care.

• Prepare and dispense intravenous admixture solutions using aseptic technique in a sterile environment.

Required

• Graduate of an ACPE accredited College of Pharmacy

• B.S in Pharmacy or Doctor of Pharmacy (PharmD) degree

• Current licensure as a Pharmacist in practicing state

Preferred

• Previous pharmacist experience
