Position Summary

Lowell Community Health Center is opening a new pharmacy and seeking motivated pharmacists to support our values and mission, while providing a positive work environment to support professional growth and development. As a pharmacist at Lowell Community Health Center, you are a member of the healthcare team. Our pharmacists are responsible for promoting patients’ understanding of their medication and provide supervision to ensure accurate dispensing at our onsite pharmacy.

Essential Responsibilities and Duties

  • Provide timely care through our onsite pharmacy
  • Process incoming new prescription, refills and transfers
  • Ensure accuracy, safety and efficacy of dispensed prescriptions through drug utilization review, interpretation of provider’s order and prescription verification.
  • Contribute to providing timely, quality care through collaborating and assisting team in tasks as needed, including answering phones, counting prescriptions, ringing out prescription orders and any other functions required to provide quality care and service
  • Organize and direct pharmacy workflow to provide exceptional care
  • Act as Pharmacist in Charge, supervise pharmacy staff engaged in prescription preparation and labeling; verify order entries and changes
  • Maintains safe and clean working environment by complying and enforcing procedures and policies
  • Initiates prior authorization requests and assist to resolve medication billing problems
  • Communicates with providers for prescription clarification and to make recommendations
  • Patient Counseling and Understanding of their Medication
  • Provide medication counseling to the patient to ensure accurate understanding of treatment, duration, instructions for use and common adverse side effects
  • Ensure medication safety through educating patients on proper storage and disposal
  • Recommend and administer vaccines
  • Identify patients with barriers to medication adherence and assist them with getting additional help to reduce those barriers
  • Collaborate with Staff to Improve health Outcomes
  • Utilize professional judgement to review medication interactions, contraindications and other precautions, then make appropriate recommendations to increase quality of medication treatment
  • Exhibit corresponding responsibility in regard to controlled substance prescriptions
  • Providing pharmacological information by answering questions and requests from other health care professionals
  • Serve as Mentor in the Professional Development of Pharmacy Staff and Interns
  • Instruct, supervise and educate staff and interns in a manner that will benefit their learning
  • Participate in training of staff and reinforce policy and procedure
  • Operate in Compliance with all Governing Bodies
  • Work within regulations, policies and procedures to maintain compliance within pharmacy operations, 340B program, security and medication disposal
  • Complies with state and federal drug laws as regulated by the Board of Pharmacy, DEA, FDA, HRSA, and 340B Program guidelines
  • Complies with all Lowell CHC organizational policies

Customer Service – Following the health center’s mission, the Per Diem Pharmacist shall provide caring, quality and culturally competent health services to all customers. This may include responding promptly to customer/patient need; responding to requests for service and assistance; solicit customer feedback to improve service; managing difficult or emotional encounters or asking for assistance to mediate.

Job Knowledge – Competent in required job skills and knowledge; exhibit the ability to learn and apply new skills; keeps abreast of current industry developments; requires minimal supervision; displays understanding of how job relates to others; maintains confidentiality per policy and regulation; fluent in job related software and systems (Pioneer, EPIC, Microsoft Office applications, wholesaler and reverse distributor systems); uses resources effectively; pursues training and development opportunities; keeps technical skills up to date; completes all organizational trainings; basic understanding of the 340B program and 340B guidelines.

Qualifications – in order to perform this job successfully, the individual must be able to complete all essential duties satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.

Education and Experience – Pharmacy degree from accredited college of pharmacy; annual completion of required continuing education credits

Certificates, Licenses, Registrations – State pharmacist license in good standing, current CPR certificate, certificate to administer vaccines

Physical Demands – The physical demands described here must be met by the employee to successfully perform the essential functions of this job. The employee may need to stoop, kneel or crouch. Lifting or moving objects up to 25 pounds or more may occasionally occur. The ability to adjust vision and focus is required.

Work Environment – The work environment is fast paced. Noise level is usually moderate. Accommodations may be made to individuals with disabilities. While performing the duties of this job, employees may be exposed to loud noises.
