Responsibilities

  • Knowledge of professional medical practice and the ability to give and evaluate patient care. Knowledge of organizational policies, regulations and procedures to administer patient care.
  • Knowledge of medical equipment and instruments to administer patient care. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in applying and modifying the principles, methods and techniques of medicine to provide on-going patient care. Skill in taking medical history, assessing medical condition and interpreting findings. Skill in preparing and maintaining records, writing reports and responding to correspondence. Skill in developing and maintaining quality services and quality improvement program. Skill in establishing and maintaining effective working relationships with patients, medical staff and the public. Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to communicate clearly.

Certifications and Licensures

  • Board Certified or Eligible in Family Medicine
  • Active State of Michigan License or eligible to apply
  • Michigan DEA or eligible to apply
  • Life Support Certification

Required Education

  • Successful completion of an ACGME Approved Residency
  • Graduation from an accredited medical school

Compensation

Base salary with Production Bonus compensation plan. Benefits include:

  • Relocation Allowance
  • Liability insurance for malpractice, covering acts and services provided in the carrying out of official duties as assigned by EMPLOYER.
  • Full health insurance coverage including medical, vision and dental shall be provided for the PHYSICIAN and dependents. Coverage will be provided through the corporate approved plan
  • Disability insurance (short-term and long-term) for the PHYSICIAN as available through the approved corporate plan.
  • Group Term Life insurance for the PHYSICIAN as available through the approved corporate plan.
  • The base allowance of paid time off (“PTO”) for each contract year of two hundred (200) working hours, after which point provider will follow PTO schedule designated in Employee Handbook. Time off shall include time off for continuing medical education (“CME”). PTO hours will be deposited into PHYSICIAN bank at the beginning of each contract year, and may be used for vacation, CME, sick and/or personal time. Compensation for PTO/CME is at base compensation rate.
  • PTO bank hours which are unused may rollover at the end of the contract year, up to a maximum of four hundred (400) hours. PHYSICIAN may also “cash-in” up to eighty (80) unused hours (paid at the base compensation rate) at the end of the contract year
  • Administrative approval, especially for absences exceeding two consecutive weeks, must be received prior to using PTO hours for vacation, CME or personal time. Time will not be added to the PTO bank for weekend CME.
  • Additional paid time off for all EMPLOYER approved Holidays. If PHYSICIAN has call on a designated Holiday, equivalent hours will be added to PHYSICIAN’s PTO bank.
  • CME allowance of four thousand dollars ($4,000) per calendar year to attend approved educational courses and seminars, subject to PHYSICIAN’s presentation of receipts. These funds are only for direct CME registration and materials and/or travel and lodging expenses for the PHYSICIAN. CME funds may be carried over in a subsequent year, up to a maximum of eight thousand dollars ($8,000) in any one year. Time off for attendance of CME meetings and workshops is allowed as approvable time under PHYSICIAN’s PTO bank.
  • A tax-deferred investment plan consistent with the terms of the Employer’s Provider Fringe Benefits Policy.
  • Fully paid annual professional memberships, dues, and licenses, limited to: (a) local level medical society, (b) State Medical Society, (c) National Medical Society, (d) one professional membership of choice (e) Michigan State initial and renewal medical license fees, (f) DEA registration, (g) Michigan Board of Pharmacy license, and (h) hospital staff dues for MidMichigan Health.
  • EMPLOYER’s required contributions for FICA and MESC Payments.
  • Mileage expense reimbursement of the current corporate approved rate for necessary work-related travel. Routine travel to and from PHYSICIAN’s assigned clinic is not included.

Production Bonuses. PHYSICIAN may receive additional monthly compensation based on the following requirements and formula.

  • Physician patient care hours as required by Physician’s Employment Contract, less approved PTO hours = Physician’s Total Hours Required per Month.
  • Total Hours Required per Month x required two (2) Patient Per Hour = Bonus Threshold.
  • Physician’s will receive $40 for each patient encounter over the Bonus Threshold calculation, less any open/incomplete patient charts.
  • Any monthly Production Bonus paid out will be capped at and may not exceed three thousand dollars ($3,000).
  • During any year when Physician’s Base Salary plus the total of any monthly Production Bonus payments received exceed the Medical Group Management Association (MGMA) ninetieth (90th) percentile of published compensation levels for providers of like specialty, the Physician will no longer be entitled to receive additional monthly Production Bonus payments under the formula set forth above.

Base Salary Adjustments.

  • Physician’s Base Salary will be adjusted via annual review process conducted by Physician’s supervisor.
  • Base Salary adjustments will be calculated based upon the Employer’s current approved rating scale for percentage increase(s) to base salary.
  • Following the contract date, any base salary adjustment will become effective on Provider’s hire date, or other agreed upon date.

Additional Patient Hours. Compensation for patient contact hours worked over the base contracted hours of forty (40) hours shall be compensated at the base hourly rate.
